Permission and Release Agreement:

I hereby authorize/consent myself and/or my child to participate in the above-named Kidcreate Studio activities. I agree not to hold Kidcreate Studio or its representatives responsible for any and all injuries sustained/suffered by the below-named participants during the activities. I also agree to waive any and all rights and claims for damages that I believe I may have against Kidcreate Studio as a result of participation in the activities.

I also authorize/consent Kidcreate to use any photos, images or likenesses taken of myself and/or my child while at a Kidcreate Studio or at a Kidcreate class for presentation under any legal condition, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. I understand that there shall be no payment for this release. I understand that I may revoke this authorization at any time by notifying Kidcreate Studio in writing.
